Used to open an account by an individual.

Used by trusts or estates, business entities, IRC501(c) (3) organizations, or state/local governments to open an account.

Change your address, your beneficiary's expected enrollment year, sample participating institutions, add telephone purchase privileges, or enroll in the automatic purchase plan.

Add, change, or revoke the designation of another individual to own your Independent 529 Plan Tuition Certificates upon your death.

» Rollover of Account Funds (PDF, 60KB)
Use this form to request a rollover from another Qualified Tuition Program, transfer an existing Independent 529 Plan account to another beneficiary, or transfer from a U.S. Savings Bond or Coverdell Education Savings Account.

» Redemption Withdrawal (PDF, 73KB)
Use this form to redeem prepaid tuition certificates at an Independent 529 Plan member school.

» Refund Withdrawal (PDF, 56KB)
1. Withdrawals for your beneficiary's qualified higher education expenses at a non-member school, 2. Non-Qualified withdrawals, or 3. Withdrawals due to death, disability or scholarship.

» Form for Physician's Certification of Beneficiary's Permanent Disability (PDF, 51KB)
Complete this form, in addition to the Refund Withdrawal form, if the Beneficiary is permanently disabled.
